School Schedule – Fall 2009 Teachers and Classrooms List Fall 2009 Registration Form – Fall 2009 In order to minimize chaos at the beginning of the semester, please turn in your completed registration form along with the check to the teacher of your child. Please remember one form and one check per child. Make check payable to CSR. Your child's teacher will only accept the registration form and check for the first two weeks of the semester. After that, you must register at the office and be subject to $10.00 late fee per child (NO EXCEPTION). You can also mail the registration form and check to the CSR mailbox at: Chinese School of Rochester , P.O.Box 18153, Rochester, NY 14618 within 2 weeks of school opening (post mark).
